Seniors are often faced with chronic or catastrophic circumstances
that stress their coping skills, personal/financial resources, and
learning and functional abilities. This can be a daunting experience
for the elderly person and their family. One major role of a geriatric
community case manager is to empower seniors to maintain healthy
independence. Case management requires a thorough, systematic assessment
of the individual's functional skills, cognitive status, limitations,
needs, strengths, abilities and available personal and community
resources.
Case managers identify at-risk seniors' (age 60 and older) needs
and develop and coordinate a plan of care by collaborating with other
health care professionals and using available community resources
to best meet their needs. They assist in activating a plan of care
and evaluating the outcomes. Re-evaluation is ongoing as the client's
status changes.
Case Managers assist with referrals, such as:
• In-Home Help, Personal Care or Housekeeping
• Meals on Wheels
• Medicare, Medi-Cal Information
• Assisted Living Care
• Physician, Attorney Referrals
• Adult Day Services
• Senior Centers: Meal and Activity Programs
• Adult Protective Services
• Financial Abuse Counseling
• Behavioral Health Referrals
• Transportation Information
• Medical Insurance Counseling
